Acquired this side mount the other day and a Google search came up with nothing. The base is curved for a rounded receiver, not flat and there's no markings or stamps. The three hols are 1-1/4" apart and almost looks like it was for an early Remington model 5 or 8 self loader. Has only one ring for a 1" scope which seems odd.
Does this look familiar to anyone?

one half a weaver base and ring..

Might have been one half of a two piece setup but not sure. I know I've seen pictures in the past of the same on a rifle using one ring.
